Most membership forms collect too much and still miss the fields the club needs in an emergency. This template collects exactly what a club uses: identity, contact, emergency details, medical flags where the sport warrants them, and the consents that keep the club on the right side of data rules.

Making it yours
Collect only what you will use
Every extra field costs joiners and creates data you must protect. If nobody at the club will act on a field, delete it.
Junior members change the form
Parental consent, collection arrangements and photography permissions become mandatory. The template's junior section covers them; delete it for adult-only clubs.
Say what happens to the data
One honest sentence about where details are stored, who sees them, and when they are deleted. It builds trust and it is what data law expects.
