Make joining short and useful
A member form must work for the joiner and the incorporated society. This shared template asks for identity, contact, category, emergency and relevant health information, with consent sections for data, images and younger members. Cut anything the club will not use. Then explain which authorised people see the details and when records are removed.
What is in the document
- Member name and contact information alongside the requested category
- Emergency details available to the volunteers who may genuinely need them
- Relevant medical questions presented as optional where that is appropriate
- Data, photography and parent or guardian consent blocks for adaptation
- A declaration and signature area for either paper or online joining
Making it yours
Remove questions without a job
If the society will never act on a field, delete it and avoid storing unnecessary personal information.
Rewrite for young members
Add the parent permissions, collection arrangements and image choices used by your junior programme.
Be plain about storage
Tell people where the record lives, which roles can access it and when the society removes old information.
Questions
What should a New Zealand club form include?
Ask for member identity, contact details, category, emergency contacts, relevant medical information and necessary consents. Include the appropriate parent or guardian section for younger members. Remove fields the society cannot connect to an actual purpose.
Can our society collect this online?
Yes. The shared document can guide a digital form or be printed for paper joining. Online use should keep each consent with the member record and restrict access to authorised society roles.
Is the template ready for New Zealand?
It is a free shared starting point, not a dedicated New Zealand form. Downloading requires no signup. Adapt the questions and data explanation for the incorporated society, then check any requirements that apply to your activities.
